Mostly solid, just technicalities to work out :) You don't need to list the damage under the technique's effects. You can actually drop the 'effects' line altogether since you're going pure damage on these. 

 

Your JP is a little off too. On the Chuunin variant, 72CP puts you in the 3JP range. Evolving still has to be a minimum of 1JP, so it'd be JP: 3 [Evolving 1]. On the Jonin, 180 Chakra lands you in 5JP territory. The Sennin comes in at 7JP, but may I ask why the numbers? 400 CP = 600 Damage is a pretty elegant ratio, much more so than 406.666666 = 610 Damage. Not going to stop it from being approved, but it's a little unusual.

 

Finally, when evolving a jutsu you need to list in the technique what it evolves from. For example, [Evolving 1 l Fantomuigunaito no Jutsu] for the Chuunin version and so on.

As for the numbers, I was trying to maximize the damage while attempting to give all the costs and such a try. Still trying to understand it all
